Trial Design

Randomized, double-blind, vehicle-controlled, multicenter, parallel-group

Duration

6 months of treatment, followed by 3 months of washout

Subjects

346 men and women aged between 30-70 years, with photoaged skin defined by the presence of fine lines around the eyes.

Regimen

Applied Isotrexol to the entire face and hands, including the wrists, once daily in the morning after washing and drying.

The standardized moisturizer (Neutrogena Liquid Soap) and sunscreen (L'Oreal / Garnier Ombrelle) could also be applied to the face and hands as required, but not shortly before or after application of Isotrexol.

Male subjects could continue to use their regular (non-mentholated) shaving cream, but not after-shave lotion or colognes on the face.

No other topical treatment was permitted throughout the study.

Methods of Assessment

Optical profilometry measurements of skin surface replicas around the eyes.

Subjective assessments of wrinkles and fine lines using a visual analogue scale (VAS) meter.

Number of actinic lentigines (age spots).

Histopathology of skin biopsies obtained from the back of the wrists.

Investigator evaluations of the change in overall skin condition.

Side Effects

Primarily irritation, including erythema (skin redness), peeling, dryness, burning sensation and itching. However, the majority of the symptoms were mild and intermittent, and the incidence of these adverse events decreased over time, with very few reports towards the end of the study.

Conflicts of Interest

2 of the 6 authors were employees of Stiefel International, which manufactures Isotrexol.

Effects observed in this study:

Wrinkle depth and skin roughness were both significantly improved with the Isotrexol cream. Women aged above 50 in particular showed notable improvements in the appearance of fine lines and wrinkles, and the improvements were maintained even 3 months after treatment was discontinued.

The colour of the skin around the eyes were assessed to have improved according to clinical scores, with the difference becoming statistically significant in favour of the Isotrexol treatment at 6 months.

The subjects also experienced reduced skin yellowness throughout the study, demonstrating a lightening of the skin, though this was not as pronounced as the improvements for wrinkles and fine lines.

Mean epidermal thickness was increased in subjects treated with Isotrexol, but whether a significant difference existed between groups was not discussed.
