Trial Design

Single-center, randomized, investigator-blind, parallel-group, uncontrolled

Duration

24 weeks of treatment

Subjects

42 women with a mean age of 50 years, of Fitzpatrick phototypes III or IV and with severe or very severe photodamage on their décolletage, with large areas of solar lentigines and moderate or severe wrinkling

Regimen

Applied a dime-sized amount of Obagi ELASTIderm Décolletage Skin Lightening Complex first, followed by 2 pumps of Obagi ELASTIderm Décolletage Wrinkle Reducing Lotion to the entire décolletage every morning and evening.

Applied either 0.025% or 0.05% tretinoin cream in the evening, pre-mixed with the Obagi ELASTIderm Décolletage Skin Lightening Complex.

Used regular body cleansing products and a speficied moisturizer (CLENZIderm MD Therapeutic Moisturizer) as needed.

Methods of Assessment

Overall assessment: Investigator's integrated assessment of décolletage photodamage on a 6-point scale and patient satisfaction rating of overall improvement

Outcome measures: Investigator's assessments of tactile roughness, mottled hyperpigmentation, lentigines, fine wrinkling, skin laxity (looseness), crepiness and coarse wrinkling on a 6-point scale

Side Effects

Side effects included erythema (skin redness), dryness, peeling and burning/stinging sensations, which were mild and transient, resolving with continued treatment.

Conflicts of Interest

Both authors have ties to Obagi Medical Products. The first author is a consultant for the company, and the second author is an employee and holds stock options.

Effects observed in this study:

Overall improvement: Photodamage of the décolletage area was significantly reduced from week 4 onward. The proportion of subjects who achieved at least a 2-point improvement (e.g. from severe to mild, or moderate to minimal) was 35% to 39% . Moreover, this proportion was still rising at the end of the study, suggesting that further improvements would be seen over longer timeframes.

Anti-wrinkle effect: Fine wrinkling was significantly reduced starting from week 4, and crepiness and coarse wrinkling were significantly decreased from week 12 onward. At the end of 24 weeks, all subjects no more than mild levels of fine wrinkling. 71% and 45% of subjects also experienced a 1-grade improvement in crepiness and coarse wrinkling, respectively.

Lightening effect: The intensity of mottled hyperpigmentation and lentigines decreased from week 2 onward When treatment was complete 79% of subjects had at most minimal levels of mottled hyperpigmentation, and 87% had experienced at least a 1-grade improvement in the appearance of lentigines.

Smoothing effect: Tactile roughness was significantly reduced after just 2 weeks of treatment. By the end of the treatment, 97% of subjects had at least a 1-grade improvement, and 76% had at least a 2-grade improvement.

Tightening effect: Skin laxity (looseness) was found to have declined starting from week 8. Post-treatment, 68% of subjects had experienced at least a 1-grade improvement.
